Spinal and Peripheral Arthropathy
A 40 year old male, otherwise an avid foot player in past, quit the game as he developed back pain followed shortly by pain in knee joints.
Sports doctor noticed skin pigmentation developing over years especially in ear. He tested positive for urine sugar as well
Treatment with multiple pain killers and steroids failed to bring significant improvement. How would you go about this case?

Is it ochronosis....with wide areas of calcification in the intervertebral discs and degenerative changes in the peripheral joints...and bluish brown pigmentation of ear (and nasal) cartilage. his urine should be tested for homogentisic acid.
